The Case Management System

Case management services represent the "heart and soul" of our client services. Case managers work on a one-to-one basis with clients, assessing each individual's particular needs, and linking them with the needed services, including HIV primary health care, OB/GYN services, substance use treatment, child care, and permanency planning.

Case managers serve as advocates. They assist clients to navigate the complicated public benefits systems and obtain benefits, such as Medicaid, SSI/SSD, housing, support groups, food programs, legal services, ADAP, and other services for which they are eligible.

In short, our case management program sees clients through the entire course of their illness, making sure their needs - be they short-term, emergency, or long-term concerns - are met in a coordinated and comprehensive manner. We provide specialized case management services to families, individuals, undocumented immigrants, people with mental illness and men at the Arthur Kill Correctional Facility.
